A U.S. person (including a U.S. citizen living outside the United States) or U.S. entity should submit  Form W-9. A non-U.S. individual should submit Form W-8BEN, and a non-U.S entity should submit Form W-8BEN-E. Complete all mandatory fields and sign where required.
